The Chronic Disease Self-Management Program (CDSMP) is an evidence-based workshop developed by Stanfod University. The workshop consists of six sessions that meet weekly for 2.5 hours.  Peer leaders facilitate the workshop, where participants learn skills to help them manage their chronic conditions.

NAPCA Healthy Aging published a report highlighting our implementation of the Stanford Chronic Disease Self-Management Program (CDSMP) in AAPI communities. To download the report, click here.

NAPCA collaborated with local community-based organizations to offer in-language CDSMP workshops for AAPI elders.

This project was funded by the Administration on Aging and Walmart Foundation.
